SECTION 3: SITE / STRUCTURE INFORMATION(required fields are marked)

If the 3.1a Site Type is:

  • Commercial:  enter the business name or facility name in 3.1b.
  • Oil or Gas: Enter the well name, API/well ID, lease name, or rectifier number in 3.1b.
  • Industrial or Utility Site: Enter the facility name or other identifying information in 3.1b.
  • For all other Site Types leave 3.1b blank unless additional identifying information would be helpful.

I am aware that ALL septic systems in Burleson County require a permit.
