Use this form to verify an existing physical (9-1-1) address. If you need a new address, please use the New Address Request form.

  • Enter the first and last name of the person or business representative whose name should appear on the Address Verification Letter.
  • If you are submitting this form on behalf of a client, enter the client's first and last name as the Address Verification Letter recipient
  • If the Address Verification Letter should be issued to a business, enter the recipient's first and last name and the business name in the Business Name field.
  • Your email address will be used to send you a copy of the Address Verification Letter.
